Consent Wavers
I, (the parent/legal guardian of the above registered minor or legal Adult), agree that I and the participant will abide by the rules and regulations of the Sebasticook Valley community Center (SVCC).Recognizing the possibility of physical injury associated with sport programs and activities, I hereby release, discharge and/or otherwise indemnify the SVCC, it affliated organization and sponsors, their employees and associated personnel, including the owners of fields and facilities utilized for the programs, against all claim by or on behalf of the registrant as a result of the registrant's participation in the program and/or while being transported to or from the same, which transportation I hereby authorize.

Photo Release
On behalf of (minor registered above or myself), consent that the photographs, artwork, audio, video, or writing submitted may be used by SVCC, it's assigned successors, in whatever they desire including television, ads, CD ROMs, online and print publications, and any other form of retrieval, and reproduction of information and images; furthermore, I hereby consent that such information, photographs, and videos, as well as plates and/or tapes from which they are made, shall be SVCC property, and they shall have the right to sell or duplicate, reproduce and make other uses of such information, photographs, videos, recordings and plates as they may desire, free and clear of any claim whatsoever on my part.

Parental Support
SVCC runs their successful programs with the help of many volunteers. Please list any ways you are willing to help. Partial list of possibilities include; coach, asst. coach, score keeper, timer, kitchen help, snack shack help, admissions, bingo, field work, official, set-up at events, take down at events, chaperon, transportation, and other.
